Person Centred Planning

Person Centred Planning is an action-oriented, individualized approach to planning, which is reflective of Community Living Stratford and Area’s philosophies and beliefs, with the emphasis on community inclusion and building relationships.  Person Centred Planning focuses on the person by putting him/her in charge of defining the direction for his/her life.  Its purpose is to look at what the person’s life looks like now, what the person’s dreams for the future are and to clearly define who will help the person take the necessary steps to make these dreams come true.  Person Centred Planning involves the development of methods and resources by which people choose their own pathways to success.  Every person will have a unique Life Plan, distinctive to his/her wants and needs.  In all cases, plans are working documents, to be implemented and evaluated on a daily basis.

Values and Beliefs

The foundation of Person Centred Planning is based on the following values and beliefs:

1.  Inclusion: Everyone has the right to be a participating member of their community.

2.  Citizenship: Rights and responsibilities, valued roles, and choosing from a range of opportunities that enable one to learn, explore and participate.

3.  Self-determination: Everyone has the right to choice and control over their life.

4.  Community as a first resource: Much of what people want out of life can be found in the community – opportunities for jobs, volunteering, recreation, relationships and experiences.  The community is rich in possibilities. 

5.  Contribution and Participation: Building on the gifts and talents that everyone has will lead people to discover their own unique way to contribute and participate in the community.

The Plan

Part of the Person Centred Planning process is the development of a Life Plan.  This is a working document that will be front and centre in the person’s life.  What this plan looks like will be individualized and different for everyone.  However, there is basic information that each plan will include:

1.  An introduction of the person and the things that others like and admire
     about him/her.
2.  A relationship map that describes the people who know and care about
     the person.
3.  The person's strengths, interests, choices and preferences.
4.  What others need to know or do to support the person best in
     relationships, things they like to do, and how to stay safe and healthy.
5.  Characteristics of people who best support the person.
6.  What makes sense in someone's life and needs to stay the same.
7.  What does not make sense and needs to change.
8.  Goals for the future.

 ***Having a plan is not the desired end result.  It is a first step.***

Focus

Our focus is on meeting the needs of individuals who choose to use our support. We structure our services based on the dreams, needs and wants expressed by people through the Person Centred Planning process. The services and supports we provide are constantly changing and evolving as we respond to new needs and wishes that people express to us. The Values and Guiding Principles below also guide us in shaping the support and services that we provide.

Guiding Principles

We believe that all persons are unique and all planning around their lives starts from a position of dignity, respect and caring for the individual.

We believe all people have the right to take risks in making choices with regards to their destiny. Our responsibility is to ensure that people are supported to make informed choices based on experience.

We believe all people have a right to belong and participate in their community. We will encourage and support relationships with family, friends and society.

We believe that all people grow and change. We participate in this by continually being open to their needs and desires, spoken or unspoken.
